How Does Hospice Care Work?

Primary Caregiver
This is usually appointed by the patient to help make medical decisions on the patient's behalf, and is typically an immediate family member.

Hospice Team
Our staff develops a Plan of Care that is tailored to and meets each patient's individual needs for pain management and symptom control.

Hospice Staff
We at Platinum Hospice Care will make regular visits to assess and care for the patient, or provide other services as needed.
